For In-Person Appointments:

Please contact the clinic at least 24 hours BEFORE your scheduled appointment to inquire about in-person availability. Not all providers offer in-person appointments every day.

Please do not attend an in-person appointment if you or a member of your household have:

New onset of sore throat, cough, chills, body aches, headache, shortness of breath, loss of smell and/or taste, fever, nausea, vomiting, or diarrhea

+

Been in close proximity to a person who tested positive for COVID-19 in the past 14 days

+

Tested positive for COVID-19 in the past 14 days
