For In-Person Appointments:

Please contact the clinic at least 24 hours BEFORE your scheduled appointment to inquire about in-person availability. Not all providers offer in-person appointments every day. |

In accordance with Public Health Madison & Dane County recommendations, masks will be required for ALL individuals. |

Please do not attend an in-person appointment if you or a member of your household have: |

New onset of sore throat, cough, chills, body aches, headache, shortness of breath, loss of smell and/or taste, fever, nausea, vomiting, or diarrhea |

Been in close proximity to a person who tested positive for COVID-19 in the past 14 days |

Tested positive for COVID-19 in the past 14 days |
Visitor Policy:
ADULT PATIENTS:

One (1) support person may attend clinic appointments with an adult patient. |

The support person must follow all COVID-19 guidelines. |
PEDIATRIC PATIENTS (under the age of 18):

Two (2) parents/guardians are allowed. |
* |
Patient siblings are NOT allowed as visitors. |

The parents/guardians must follow all COVID-19 guidelines. |
